Your First Visit
We ask that you arrive at least 30 minutes before your scheduled appointment to complete any necessary paperwork. You can save time by completing our patient registration forms online. Please be sure to bring the following information to your appointment or you may email them to: [email protected]
- Current X-Rays (less than 2 years old)
- A list of medications that your currently taking
Please alert the office if you have any significant medical history such as allergies, was told to pre-medicate prior to your visit, or had an artificial heart valve or joint replacement. The goal of your first visit is to establish a vital foundation for our relationship with you. During the first visit, we make sure to obtain implant background information (ie: medical and dental history); wishes, dreams, and desires for dental care; and give you time to get to know Dr. Rolf and the staff.
What to Expect
Being well-prepared for your appointment will ensure that Dr. Rolf has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this Web site. Your initial oral examination includes a visual examination, charting, periodontal probing, diagnosis, and treatment recommendations when indicated. We will also take appropriate digital x-rays, that may include a panoramic x-ray for proper diagnosis of the anterior (front) and posterior (back) teeth as well as a bite-wing x-ray series for proper diagnosis of proximal decay of posterior teeth. Please expect a longer appointment on your first visit. We look forward meeting you. We feel confident that after your first visit, you will know you have made the right decision in selecting us.

There will be a charge for any appointments cancelled less than 48 hours.
